Checklist for online submission of Application Form :
** Carefully read important instructions and information given in the ‘Admission Bulletin’ before filling the application form. This is available as a free download on the Admissions Page , above the ‘Apply Online’ icon.
** Create an email ID if you do not have one. All communication will be sent by email only. It is your responsibility to ensure that the email id is correct.
** You must keep a soft copy of your photograph and signature ready before filling the application form as you need to upload it during the application process.

Specification for photograph: Recent colour passport size photograph, light background, jpeg format not exceeding 80 KB (Width – 150 pixels & Height – 150 pixels).
Specifications for signature: Scanned signature, jpeg format not exceeding 50 KB (Width – 150 pixels & Height – 50 pixels)

** The application fee includes a basic administrative fee of Rs.800/- in addition to a registration fee per course applied. The registration fee per course is given below,
Group A courses:
MBBS, BSc (Nursing) & AHS Degree Courses – Rs. 500/- per course
Group B courses:
Diploma in Nursing and other Allied Health Science diplomas – Rs. 100/- per course
MSc courses and Fellowship in Analytical Clinical Pharmacology – Rs. 500/- per course

** The application fee can be paid as a Demand Draft (DD) at any scheduled bank OR as a challan in the ICICI bank OR by using a credit / debit card. If you are paying by DD, you must keep the DD ready before filling the application as you will have to enter the DD number during the application process.

** This is a common application form for both Group A & Group B courses. You can apply for courses in Group A or Group B or both. There should be only one online application submission per candidate

** Please note that this entire application form is to be submitted online. If the application process gets interrupted after you submit the basic information or at “upload photo and signature”, you can sign in again using your application number and date of birth. If you did not note your application number, you must go to the main menu & follow the procedure to “Retrieve Application Number.”

** Please do not send the hard copy of your application form to the Registrar’s Office. If the fee is paid by Demand Draft or ICICI Challan, then the original Demand Draft or ICICI Challan alone has to be sent to the Registrar’s Office on or before 1st April 2016.
** While filling the form, if you get a pop-up message which says ‘prevent this page from creating additional dialogs’, DO NOT click in the box next to this message.
