The application Form can be obtained (a) From University Office or (b) by downloading from University website sumandeepuniversity.co.in, and is to be submitted along with Demand Draft of Rs.3500/- (Form Fee + Test Fee) in the name of Sumandeep Vidyapeeth, payable at Vadodara till 28/11/2015 and application form submitted after 28/11/2015 be submitted with Demand Draft of Rs.4000/- (Form Fee + Test Fee + Late Fee) to the Admission Cell in person by 3rd December, 2015.

ELIGIBILITY :
(A) MEDICAL :
** The candidate must be an Indian National.
** The candidate must have passed M.B.B.S. Examination from a recognized Medical College and should have obtained registration either from the Medical Council of India, New Delhi or State Medical Council after completion of one year rotating internship training.
** The candidate undergoing one year rotating internship training and likely to obtain final registration certificate from the Medical Council of India, New Delhi or State Medical Council on or before 1st March, 2016 can also appear for the Entrance Test.
** Such candidates will be required to submit a certificate from the Principal/ Dean of the College regarding the likely date of completion of their rotating internship training failing which their application will be treated as incomplete and shall be rejected.

(B) DENTAL :
** The candidate must be an Indian National.
** The candidate must have passed B.D.S. Examination from a recognized Dental college and should have obtained registration either from the Dental Council of India, New Delhi or State Dental Council after completion of one year rotating internship training.
** The candidate undergoing one year rotating internship training and likely to obtain final registration certificate from the Dental Council of India, New Delhi or State Dental Council on or before 1st March, 2016 can also appear for the Entrance Test.
** Such candidates will be required to submit a certificate from the Principal/Dean of the College regarding the likely date of completion of their rotating internship training failing which their application will be treated as incomplete and shall be rejected.

International Admissions:

** 15% of the seats allotted for each specialty are reserved for Foreign Nationals/Persons of Indian Origin/Non-Resident Indians. The candidate in this category will have to appear for AIPGMDET- 2016.
** The candidate seeking admission under this category should have registration with either MCI or DCI as applicable.
** In case of non-receipt of applications / absence of eligibility, these seats will be allocated to General quota students as per merit.
** The candidate should apply for AIPGMDET-2016 along with the copies of certificates listed in the application form and the required fees.
** Produce the document of provisional admission to the Indian Embassy in the respective country and get a student visa endorsed to institution.
** Report to the institution for admission. Fill the permanent enrollment form and submit it along with the following documents (in original along with a Photocopy) : Mark / Grade statement of the qualifying examination.
** Transfer certificate from the institute last attended and migration certificate should be submitted along with the application.
** A Photocopy copy of their passport – duly attested by a notary.
** Physical fitness certificate from a registered Physician.
** Admission of international students will be confirmed only after verification of original certificates, medical fitness test and payment of required fees.
** Within a week of arrival in India, register with the police in the Foreigner Regional Registration Office (FRRO) of the local Police.

A candidate desirous of appearing for All India Post Graduate Medical and Dental Entrance Test of this University will be required to submit application in the prescribed form along with the attested copies of the documents in support of the application.

Entrance Test Fee:
** The fee is payable by a demand draft, drawn in favor of “Sumandeep Vidyapeeth” of nationalized / schedule bank, payable at Vadodara. The candidate should write his/her Name, Address and Mobile No. on the reverse side of the demand draft and send to the The Registrar, Sumandeep Vidyapeeth, At & Po. Piparia, Tal.: Waghodiya, Dist. Vadodara 391746. (Gujarat) [INDIA)
** Along with application form candidate has to submit DD for Rs.3500/- (Rs.1000 + Rs.2500/-) before 28.11.2015 thereafter upto 3.12.2015 Rs.4000/-
** The last date of receipt of completed application form along with requisite documents and entrance test fee at the office of Registrar, Sumandeep Vidyapeeth Piparia upto 2.00 p.m. on 28/11/2015 & (with late fee of Rs. 500) up to 6.00 p.m. on 03/12/2015.
** It will be the responsibility of the candidate to ensure that his/her application reaches the office of the Registrar, Sumandeep Vidyapeeth within the specified time and date.
** Application in all respect must be addressed and sent to the Registrar, Sumandeep Vidyapeeth, At & Po.: Piparia, Tal: Waghodia, Dist. Vadodara 391 760.
** The application received after the due date or not accompanied by entrance test fee / supporting documents shall be rejected and such candidates shall not be allowed to appear for the Entrance Test.
** The university will not be responsible for non-receipts of applications within the specified time and date due to postal or courier service delay.
