5 Fee Rs. 500/- (in the form of DD in favour of Registrar, Baba Farid University of Health Sciences payable at Faridkot) for correction in online admission application form. 27.08.2015

ELIGIBILITY AND QUALIFICATIONS :
Eligibility and qualifications will be as p er Punjab Govt. Notification No. 5/6/2013-3HBIII/524462/1dated 01.07.2015.

Fee Required with Admission Application Form:
a) Rs. 10,000/- should be deposited in any branch of the Oriental Bank of Commerce throughout India by using Bank Challan form available on University web site i.e. bfuhs.ac.in by due date and time.

b) Fee once deposited in University account will not be refunded/adjusted in any case.

IMPORTANT INSTRUCTIONS AND PROCEDURE TO FILL ONLINE ADMISSION :
APPLICATION FORM FOR ADMISSION TO DM/M.Ch. COURSES UNDER SSET-2015. :
1. Before applying, candidate should carefully read PROSPECTUS for eligibility conditions and other important instructions.

2. Only one application is to be submitted by a candidate. Submitting more than one application by a candidate is liable to be rejected.

3. Candidate will deposit a fee of Rs. 10,000 in the University account through any branch of the Oriental Bank of Commerce by using specified Bank Challan Form, which will be generated after submitting online admission application form at university website i.e. bfuhs.ac.in. After depositing fee, candidates will enclose University copy of Bank Challan Form with printout of admission application form to be submitted at the venue of practical/viva. Fee shall not be accepted in any other mode i.e. cash, Cheque, Demand Draft etc.

4. Date should be filled in the format of dd/mm/yyyy i.e. date/month/year.

5. There will be a check list at the end of second page of admission application form. Please make sure that all the relevant documents are attached with application as mention in check list.

Note:
Please fill your valid mobile no. and email address to receive information regarding your application form time to time in the form of SMS and Email.

Important:
In case candidate wants to make any change in his/her admission application form, he/she has to write separate application wherein he/she will clearly describe the information that he/she wants to modify. He will also attach copy of his/her admission application form and submit his/her request personally in the University by due date (i.e. last date of application form submission) with correction/editing fee of Rs. 500/-. After it request to change information in application data will not be entertained at any cost.

Important Instructions for filling of Online Super Speciality Entrance Test 2015 :
Step 1 – Fill Registration Form :
Please fill registration form carefully and after successfull registration of application form, you will receive a Registration No. and Password on your mobile and Email. Please take print out of Bank Fee Challan form at the end of registration.

Step 2 – Deposit Fee :
Please deposit Fee in any of Oriental Bank of Commerce branch through Challan form printed by you earlier. Fee for Super Speciality entrance test is 10,000/-.

Step 3 – Upload Photo :
Please Upload your 25kb size passport size of Photograph.

Step 4 – Lock Application :
Please Lock your application only if you sure that all the information entered by you is correct. Because you can’t edit your application form information after locking it.So, make sure all these things before locking the application form.

Step 5 – Print Application :
You can take print out of your application only if your application form is locked. So, first lock your application then take print out and send it to University along with Bank Fee Challan form and other required documents in due time.

Step 6 – Download Rollno :
Please enter your Sr. and Password to download Admit card/Rollno slip on 21-08-2015

P L E AS E NOTE – Re a d c a r e f u l l y :
1. Please visit our website bfuhs.ac.in for all regular and updated information concerning admission to Super Speciality course Session-2015 and dates of counselling, from time to time.

2. For appearing in Entrance Test candidates will have to fill online admission application form and deposit the required fee in the OBC Bank and send the print out copy of online admission application form alongwith university copy of Bank Challan form and self attested photocopies of all required documents in the University by due date.

3. Fee should be deposited directly in the University account through any branch of the Oriental Bank of Commerce by using Specified Bank Challan Form which will be generated on successful filling of online admission application form available at University website i.e. bfuhs.ac.in. Fee shall not be accepted in any other mode i.e. cash, draft, cheque, etc.

4. Admit Card/Roll Numbers of SSET-2015 will be issued only to those candidates who will deposit the fee in Bank by due date and time.

5. Merely deposition of online admission form and fee, will not entitle the candidate for admission. Print out of admission application form alongwith University copy of Bank Challan and self attested copies of certificates will be received in the University by due date and time.

6. Applications received after the due date and time for any reason including postal delays will be rejected. The University will not be responsible for any delay or loss in transit caused by Postal Dept./Courier Services or anyone else.

7. Candidates are advised to fill online admission application form after checking his/her eligibility as mentioned in this prospectus. Fee once deposited will not be refunded/adjusted in any case.
