Applications can be downloaded from the website tnhealth.org and the filled application along with necessary enclosures and 2 self addressed envelopes of 23 x 10 cms size, each stamped to the value of Rs.40/- (Rupees forty only) respectively may be sent to the Secretary, Selection Committee, Commissionerate of Indian Medicine and Homoeopathy, Arignar Anna Government Hospital of Indian Medicine campus, Arumbakkam, Chennai 600 106 before the last date with a Demand Draft for Rs. 2,000/- ( Rupees Two thousand only) which is non refundable towards the cost of application form drawn in favour of the Director of Indian Medicine and Homoeopathy, Chennai 106 from any Nationalised bank, on or after $-07-2015,payable at Chennai.

** Applications without the Demand Draft will not be accepted.

** The name of the candidate and contact phone number should be written on the reverse of the Demand Draft.

** SC/SCA and ST candidates who are natives of Tamil Nadu are exempted from payment of the cost of the Application Form.

** SC /SCA /ST candidates should enclose the attested Xerox copies of the Community Certificate (two copies) and BHMS / equivalent certificate along with the filled in application form.

** Candidates should send the application with all enclosures in an A4 size cloth lined cover.

PAYMENT OF COURSE FEE DEPOSIT :
The candidates who are selected at the time of counseling shall remit a non refundable sum of Rs.5,000/- ( Rupees Five Thousand only) by way of Demand Draft in favour of the Director of Indian Medicine and Homoeopathy, Chennai 106 drawn from any Nationalised bank on or after 11-08-2015 payable at Chennai at the time of receipt of the allotment orders.

Students of SC/SCA/ST whose parent’s / guardian’s annual income is less than Rs.2,50,000/- (Rupees Two lakhs and fifty thousand only) and students of converted Christian community whose parent’s / guardian’s annual income is less than Rs.2,00,000/- (Rupees Two lakhs only) are exempted from the payment of Rs. 5,000/-. After the receipt of the joining report from the Principal of the college, this amount will be transferred to the college to get adjusted with the Tuition Fees to be paid by the candidates.

If the candidate fails to join the course after getting allotment order within the stipulated time, he/she will forfeit this amount as well as seat allotted.

In addition, the candidate selected for Post Graduate M.D (Homoeopathy) course will have to remit a non-refundable amount of Rs.500/- (Rupees Five hundred only) by means of Demand draft drawn in favour of Director of Indian Medicine and Homoeopathy, Chennai 600 106 on or after 00 – 00-2015 payable at Chennai from any nationalised bank as processing fee for attending counseling.

GENERAL INSTRUCTIONS :
1 Before filling up of the application form, carefully read all the instructions given in the Prospectus and Application form.

2 Fill the application carefully and legibly without overwriting.

3 All enclosures prescribed should be true photocopies, duly Self attested. The originals should be produced at the time of Counselling and admission only.

4 Completed applications should reach :

5 Application with incomplete or insufficient particulars or without enclosures or those received after the last date mentioned will be summarily rejected.(without any intimation to the candidate) The selection committee admits no responsibility for return of application forms.

6 Applications received by speed post/ courier or any other means after the last date and time will not be accepted irrespective of the date of booking.

7 Non-service candidate should send their applications directly to the Secretary, Selection Committee at the above address.

8 Service candidates should get their application forwarded only through the Proper channel with the remarks of the forwarding authorities in the format prescribed in the application form. If not, applications will be summarily rejected.

9 Advance copies from the service candidates will not be accepted.

10 All pages in the application should be signed in full by the candidate.

11 A Self attested passport size photograph should be affixed in the front page of the application form in the space provided. Please affix a recent photograph to enable identification in the Examination hall. Similarly, two recent passport size photographs for the identification slips should be affixed in the spaces provided. All photos on the application form should be identical.

12 If a candidate desires an immediate acknowledgement, should advise the postal or courier service and pay the required fees themselves. Intimation card confirming receipt of application with required particulars and date of examination will be sent subsequently to candidates who are found to be prima facie eligible. Hall tickets will be issued. Candidates should use theirApplication Registration Number (A.R. Number) in all correspondence.

13 All communications to the candidates will be sent only to the mailing address given by the candidate in the application and self addressed envelopes. Candidates are requested to intimate any change of address immediately. The intimation/call letters will be sent by Speed Post/Courier Service/SMS (Short Mail Service) whichever is feasible. The selection committee will not be held responsible for postal delay and wrong messages conveyed by the postal department or courier service.

14 Request for extension of time for submission of documents beyond the prescribed date and time shall not be entertained.

15 Candidates selected for admission should give a declaration in the form prescribed at the time of Counseling.

16 The candidate must write his/her Name , contact phone number and Address on the reverse side of the demand draft.

17 Both upper limbs, vision and hearing should be normal for all candidates including for candidates applying under physically disabled category.

ELIGIBILITY FOR ADMISSION – M.D. (HOMOEOPATHY) :
(i) Candidate should be a citizen of India and should have undergone BHMS Course in the State of Tamil Nadu

(ii) Candidate of Tamil Nadu Nativity who have completed BHMS in other States should furnish a Certificate of Nativity in Tamil Nadu issued by the competent authority only in the specified format in Annexure-I and supported by a copy of passport / Ration card / voter’s I.D/ Driving License / Permanent Medical Registration Certificate. If the copy of the Supporting document is not produced, then the application will not be considered. Nativity Certificate submitted later will be summarily rejected.

(iii)Candidate should possess the Degree of Bachelor of Homoeopathic Medicine and Surgery or equivalent qualification in Homoeopathy included in the Second Schedule of the Homoeopathy Central Council

(iv) Those candidates who are undergoing CRRI and will be completing their CRRI before the entrance examination should enclose a certificate from Head of the Institution in which they are undergoing the CRRI that they will be completing the CRRI before the date of Entrance Examination, failing which their application will be rejected.

(v) The candidates should have registered their names either in the Central Council of Homoeopathy Medicine, New Delhi (or) in the Tamil Nadu Homoeopathy Medical Council, Chennai. Candidates should produce the certificate before counseling.

Candidates should possess BHMS Degree of Tamil Nadu Dr.M.G.R. Medical University or of any other University recognized as equivalent to by Tamil Nadu Dr.M.G.R. Medical University and recognized by the Central Council of Homoeopathy. Candidates who have qualified from other universities should produce ELIGIBILITY CERTIFICATE from Tamil Nadu Dr.M.G.R. Medical University, Guindy, Chennai 600 032 before counseling, failing which they will not be considered for selection.
