BSc Nursing Admission Entrance Test :

Applications are invited from eligible unmarried Indian nationals (irrespective of gender) for admission to BSc (Nursing) (4-year degree course) beginning in academic year 2015-16.

Essential Qualifications :
Applicants must have passed 10+2 or equivalent examination with Physics, Chemistry, Biology and English with 50% marks for general category and 45% marks for reserved category candidates.

Age Limit :
The candidates should be between 17 and 24 years of age on December 31, 2015.

How to Apply :
** Information brochure and application form can be downloaded from the Institute’s website [http: (sgpgi.ac.in).].
** Please read the instructions carefully before filling up the form.
** The candidates will have to fill up the application form “ONLINE” beginning one clear working day after depositing the fee through State Bank COLLECT.
** Last day of depositing the fees is June 30, 2015 and filling up the form is July 2, 2015.
** After online submission, the completed application form must be downloaded, printed, signed and sent, along with documents in support of age, qualifications, domicile and caste, to The Executive Registrar, SGPGIMS, Raebareli Road, Lucknow 226014.
** The form should be sent only through registered post / Speed Post. Forms will not be received by hand or through a courier service.
** Last date of receipt of completed application forms is July 6, 2015.
** Forms received after this date will not be entertained.

Application Fee and Mode of Payment :
Through Net banking, Debit / Credit card or through computer generated challan form in any branch of State Bank of India. Application fee for all categories is 1000 + bank transaction charges as applicable. Application fee once paid is not refundable, under any circumstances.

General Information :
** Selection will be made through an entrance examination to be conducted on July 12, 2015 (Sunday) during 2:30 PM to 5:00 PM. The candidates will need to reach the examination venue at 1:30 PM.

** Reservation will be available to candidates domiciled in Uttar Pradesh (UP), as per UP Government rules. Candidates domiciled outside UP will be treated as General Category.

** At the time of admission, each selected candidate will have to pay Rs. 5000 as admission fees; Rs.10,000 as caution money (refundable on completion of course); Rs. 500 as enrolment fees; and recurring fees for the first year [including Rs. 50,000 (non-refundable) per year as course fee; Rs. 500 per year as Library fee; Rs. 2500 per year as examination fee; Rs. 300 per year as certificate fees; Rs. 500 per month (Rs. 6000 annually) as hostel fee], and any other fee as may be prescribed by the Institute from time to time at the time of admission. Food charges, expenditure on uniform, books and study material will be borne by the students as per actuals.

Candidates should enclose the following documents with the application:
Self-attested photocopies of high School and intermediate certificates and mark-sheets.
Self-attested photocopy of caste certificate, issued by the competent authority within the past 6 months (only for reserved category candidates).
Two passport size colour photographs duly affixed

** The application along with the above mentioned documents should reach Executive Registrar, SGPGIMS, Raebareli Road, Lucknow 226014 by 6/7/2015 by 5.00 p.m., through registered / speed post only in an envelope superscribed “(Advertisement number I/14/ER/2015-16)” and “Application for entrance exam of B.Sc. Nursing course”. The Institute will not be responsible for any postal delay.

** No applications shall be received by hand.

** Incomplete or late applications will be summarily rejected.
** In case of any dispute, the decision of the Director, SGPGI, Lucknow will be final.
** Director reserves the right to accept or reject any or all applications without providing any reason thereof.

** No separate call letter will be issued for appearing in the entrance test. Candidates will be required to download the admit card from the Institute’s website, as per the time schedule indicated above
