Instructions For Submission Of Online Application Form :
a) At onset of online application, candidates must have ready Demand Draft of 600/- for General & 500/- for SC,ST & PH drawn in favour of ” National Institute for the Orthopaedically Handicapped” payable at Kolkata”.

b) Candidates must have scanned copy of recent colour passport photograph within size limit of 3OKB and scanned copy of self-signature and Father’s/Guardian’s signature within IOKB(file tune should be only .ipg or .ineg or .png or .gif)

c) Option for test centers filled once cannot be changed by the candidate. However, in unavoidable circumstances the examination center may be changed by the competent authority of CET-2015.

d) Please note that name, father’s name and date of birth should as recorded as per High School or first Board/Matriculation examination certificate. Any deviation may lead to cancellation of your candidature, whenever noticed.

Upload the scanned copy of self photo with image size less than equal to 3OKB

1. NAME OF THE CANDIDATE :
** Write your name as given in your original certificate of the High School or Your first Board/University Examination.
** Write single letter in a box.
** Leave one blank box between any two parts of the name.

2. CASTE/CATEGORY: Please select the appropriate option

3. GENDER: Please select the appropriate option.

4. NATIONALITY: Please select the appropriate option.

5. DATE OF BIRTH : Enter the date, month and year of your birth as per the English calendar and as recorded in your High School/Matriculation/Secondary School Certificate.
Use numerals 01 to 31 for Date, 01 to 12 for MONTH and four digits for the YEAR of birth e.g. April 17, 1985 should be entered as follows.,

6. Center Code : Please select the appropriate Center Code as per your preference.

7. YEAR OF PASSING IOTH CLASS OR EQUIVALENT :
Candidates are required to fill up their passing year of both Class and Equivalent Examination in boxes.

8. YEAR OF PASSING 12TH CLASS OR EQUIVALENT :
Candidates are required to fill up their passing year of Qualifying (10+2) Examination in boxes.

9. MARKS IN QUALIFYING EXAMINATION (10+2) :
** Kindly fill in the appropriate subject and percentage of marks obtained in the 10+2 (qualifying examination).
** The original mark sheet will have to be produced at the time of counseling\admission.
** If the candidate is appearing in 10+2 in this academic year, then put zero in the relevant box.
** The marks should be rounded of the nearest whole number.

10. ADDRESS OF THE CANDIDATE :
** Write your complete postal address for sending communication.
** Address must include your Name, C\o Name ( if required) and Pin code which are essential for speed post to reach you.
** Candidates are encouraged to provide e-Mail address\Mobile No. for fast communication.

11. GIVE NAME OF THE PARENT/GUARDIAN AS RECORDED IN THE 1OTH CLASS CERTIFICATE

13. GIVE DEMAND DRAFT DETAILS

14. DECLARATION BY THE CANDIDATES :
The Candidate must UPLOAD scanned copy of self (use black ink only) and parents signature (use black ink only) in the appropriate place.
The image size must be less than IOKB.

FINALLY press SUBMIT BUTTON.
You will get an USER ID and PASSWORD for future login to generate admit card and to know the application status.

After submission of the form take two copies of print out. Send one copy along with DD by speed\registered post to:

INSTRUCTIONS FOR COMPLETION & SUBMISSION OF OFFLINE (Manual) APPLICATION FORM :
** Use Black ink ball pen only to fill-up the form.

** Candidates must retain a photocopy of the filled-in application form & DD, before submitting it for their personal records and future reference.

** Application form is to be filled up neatly and legitimately.

** Please ensure to writeaddress and sign the application form only in the relevant box .

** The photograph of the size 3.5cm X 4.5cm must be against a light background. Information required in the relevant box is to be filled in Capital Letters only and tick clearly the required information in the boxes wherever provided.

** Option for test centers filled once cannot be changed by the candidate.

** However, in unavoidable circumstances the examination centre may be changed by the competent authority of CET-2015.
