Instructions For Filling The On Line Application Form :
1. Candidates are advised to retain with them a print out of the application form Candidates can take a print out of the form from our web site by entering trans-action id number and date of birth.

2. Type in capital letters only and tick the appropriate boxes.

3. Select option for the Test Centre city carefully. .
4. Ensure that the candidate’s name and date of birth are the same as those mentioned in their High School or Board certificates.

5. Fill the application form completely. Incomplete application forms will be rejected .

6.Application Form after submission cannot be edited or altered therefore check all the fields and then press submit

7. Please read instructions for filling up application form available on the web site

6.1 Name of the Candidate :
Write your name in Capital letters as given in your original certificate of the High School or the Board Examination
6.2 Date of Birth :
Enter the date, month and year of your birth as per English calendar and as recorded in your High School or Board Examination certificate.
6.3 Sex :
Check the appropriate button
Male / Female
6.4 Address for Communication :
Write your complete postal address to which communication is to be sent. Also mention Tahsil and District in which the place is situated.
6.5 State :
Mention name of the state in which the place in the address to which communication is to be situated.
6.6 PIN Code :
Mention correctly PIN code of the place to which communication is to be sent.
6.7 E-Mail lD :
Use your most accessed E-mail ID. Your mail id will receive Hall Ticket and communication from AIET 2015 Office.
6.8 Tel. No. (With STD Code) :
Write your telephone number, if any, with STD code prefixed to it. Also write your mobile number if any.
6.9 Fax No. (With STD Code) :
Write your fax number, if any, with STD code prefixed to it.
6.10 Choice of Centre (City) :
Select a convenient city as your centre of the Entrance Test from the list of the centres given in the prospectus and enter it accordingly.
6.11 Qualifying Examination :
Write name of the Board from which qualifying examination is passed / appeared.
6.12 Signature of Candidate :
The Online registration form do no need signature of applicant however Applicant need to sign on the Hall Ticket in Exam Hall
6.13 Whether passed or appearing for the qualifying examination :
Please specify correctly
6.14 Photograph :
Candidate should browse the soft copy of passport size photo and needs to be submitted along with on line application form. The forms not displaying facial properties of candidates will be rejected.
The Photo MUST be in JPEG soft format with 150 by 150 PIXEL

6.15 Dr. D.Y. Patil Merit-Cum-Means Scholarships :
Dr. D.Y.Patil Merit-cum Means Scholarships scheme is applicable for MBBS and BDS course.
Specify whether you qualify the basic condition for award of the Dr. D.Y. Patil Merit-cum-Means Scholarship, and then tick mark appropriately against Yes/ No and also in the box covering the categories A/B as the case may be. For details of the scholarship, refer to para No.13.
