Note:
Apart from the above, special fees due to the college, Hostel, Uniform, amounts payable to the university / statutory bodies/Govt. Agencies, etc. are to be paid by the candidates. The same shall be remitted at the respective college before the commencement of MBBS classes.
Following Original Documents are to be Submitted to the Principal at the time of Admission:
1) SSLC/10th standard certificate/mark list.
2) Proof of the date of birth, if applicable.
3) Nativity Certificate, if applicable.
4) Certificate and/or Mark list of qualifying examination (Plus Two/12th std.)
5) Transfer Certificate (T.C.) from the institution last attended.
6) Course & Conduct Certificate from the institution last attended (Preferably issued within six months)
7) A signed copy of the printout of this Allotment Order.
8) Print out of the admit card of the NEET-2016.
9) Print our of the Result/Score Card of the NEET-2016.
10) 12th Migration Certificate, if applicable.
11) Eligibility/Equivalency Certificate of 12th from Kerala University of Health Sciences, if applicable.
12) Equivalence Certificate from Association of Indian Universities, if applicable.
13) Conduct certificate from the Head of Institution as per the government regulations on Prohibition of Ragging (Refer Prospectus 2016, clause 12.2)
14) Community Certificate from the Parish Priest / Pastor, if applicable.
Others :
1) TWO sets of attested photocopies of all the above original documents.
2) Photocopies (three) of any Photo ID (Adhar/Passport/Voters ID) of applicant attested by Gazette Officer.
3) Photocopies (three) of Permanent Account Number (PAN) Card of Guardian remitting fees for student.
4) TEN copies of Passport Size Colour Photograph (with light background, displaying name and date of birth of student at the bottom), out of which one copy shall be laminated on the front.
5) DD for First Year Tuition Fees as indicated above in favour of KCPCMF payable at ERNAKULAM.
Please note that this allotment is provisional based on the claims made by the candidate in the application. If on verification of the original documents at the time of admission, the claims are found to be incorrect, this allotment is liable to be cancelled.
The allotment is also subject to submission of all original documents to the Principal along with the remittance of Fees at the college in accordance with the date & time notified above.
